I was so fortunate to be coached by Jake and teammate of Bryan in basketball way back in the late 80's. Wonderful memories thanks to an amazing coach who poured his heart into every player. All my best to the Jacobs family.

Deanne, Kristen and family:

I was deeply saddened when I learned today of the news of Jake's passing. I hope that you all are doing OK. You are certainly in my thoughts and prayers.

I will miss the big fella. In the short few years that I got to know him, I cherished his mentorship and his friendship. I enjoyed every moment with him and learned a great deal from him.

Jake's legacy stands strong in what he has built in ADURs, and you should all be quite proud of his enormous contributions to our profession, as well as the broad-scale, downstream impact that his work helped to deliver in improving the health and well-being of countless Medicaid members all across the country.

He cert
...ainly will be missed, but his smile and warm-hearted nature will live strong in the hearts of us all whom he touched throughout his journey here on earth.

God bless him and all of you who now hole the important task of carrying on his memories for generations to come.
